Providers are encouraged to report to the Office of Inspector General (OIG) any matters involving fraud, waste, and mismanagement found in any Centers for Medicare & Medicaid Services (CMS) program(s). To assist you, the OIG maintains a Hotline that offers a confidential means for reporting vital information.

• Information is for official use only.
  To learn more about confidentiality, contact the Hotline and ask about their "Confidentiality Source Program."
• Each caller is encouraged to provide information on how he or she can be reached for additional information; the caller may remain anonymous.

To assist the OIG, please provide the following information to the best of your ability when contacting the Hotline:

• Type of complaint
  • Medicare Part-A
  • Medicare Part-B
• Referral information
  (If you prefer to submit your referral anonymously, please indicate so in your correspondence or phone call.)
  • Your Name
  • Your Street Address
  • Your City, State and Zip Code
  • Your E-mail Address, if available
• Subject, person, business, or department the allegation is against:
  • Name of Subject
  • Title of Subject (if applicable)
  • Subject's Street Address

• Brief summary relating to your allegation
